The physician review fee ($35 for single-vial prescriptions / $50 for multi-vial prescriptions, plus a $35 shipping fee) is collected directly by the independent medical group at intake and is non-refundable. The program payment is refunded in full if the physician does not approve treatment. The shipping fee is refunded only if no prescription is fulfilled.
